by fax or mail, just follow the instructions below:

the Blue
Cross application .
When
the Adobe Acrobat reader opens with the application, use
the menu bar at the top to save the application to a file
or simply print it out right from the reader. (You
may also fill out our Application
Request Form and we will send you an application
right away.)
- Simply
follow the instructions and complete the application.
- If
paying by credit card, you may fax your application toll-free
1-866-284-0082.
- If
applying by mail, send the completed application along
with a check for the $20.00 application fee made payable
to Blue Cross Blue Shield of Arizona to:
