The fastest and easiest way to apply for any plan with Blue Cross Blue Shield of Tennessee is to Apply Online, using the link above.  Gather the following information before you start the application in order to speed the process:

1. Your doctors’ names, addresses, and phone numbers
2. Medications you currently take or recently took, including dosage and frequency
3. Dates of hospitalization
4. The name of the plan for which you are applying
5. Your requested effective date
6. Your credit card information to cover the initial premium

For security, Blue Cross Blue Shield uses 128-bit Secure Socket Layer (SSL) encryption, the highest encryption available to most browsers, with a public key length of 1,024 bits.

To Apply by Mail:

1. Select the deductible option that's right for you.  The same deductible option must apply to all family members. The plan description and rate tables can help with your decision.
   
   
2. Use our instant quote system to get the total initial premium payment due.
   
   
3. Download and complete the BCBS of TN application.   You may also fill out our Application Request Form and we will send you an application right away.
   
   
4. Include your first month's premium payment.
   
   
5. Mail your completed application to us at the address below.

Your acceptance will be based on the information you provide.  Applicants must meet height and weight guidelines and minimum health requirements before the application can be accepted.  If accepted for coverage, your premium rate will be determined based on the health status of all applicants.  Various levels of rate classifications are offered.  Once accepted for coverage, your premiums cannot be increased due to changes in your health or the number of claims you file.

Once your application has been approved, you will receive an authorization form to use if you wish to set up an automatic payment through a bank or credit card draft.
