Coverage Synopsis

Coverage with the BluePreferred HSA plan comes with the value of choice.  Members have the advantage of deciding which deductible amount and coverage fits their lives and their budget. The higher your deductible, the lower your monthly premium.

When using an in-network provider, you pay nothing for child’s preventive services and female cancer screenings such as Mammograms and PAP tests. Office visits require a standard fee of $25 and Emergency Care requires $50. For services such as Hospitalization, Diagnostic Tests, and Physical Therapy, you pay your selected coinsurance percentages after the deductible is met. With the 100% plan, CareFirst will pay for 100% of covered expenses once your deductible has been reached.

BluePreferred HSA offers Vision Services and Prescription Drug Benefits with a $100 deductible and $10/generic, $25/Preferred Brand, $45/Non-Preferred Brand copayments. Vision services extend to an annual routine eye exam that requires a $10 copayment.

Optional Benefits

BluePreferred’s Prescription Drug Card Program is a 3-tier program that covers both non-maintenance and maintenance prescription drugs from a retail pharmacy. Members can use their card at 59,000 participating pharmacies.

CareFirst members have the option of adding dental and vision coverage to medical plans. You can select coverage for preventive dental care, cleanings, fillings, X-rays, and orthodontics.

Optional Maternity and Prenatal Coverage is also available through this policy. At a cost of just $126 per month added to the premium if your health policy, you receive benefits for covered pre and post-natal care as well as delivery services. (Note: If you are pregnant at the time of enrollment or apply for coverage after initial enrollment, there is a mandatory 10-month waiting period for maternity benefits.)
