This
information is presented only as a very brief overview
of some of the benefits of this plan, and is intended
only for general education. The amount of benefits
provided depends on the plan selected. Premium will
vary with the type of benefits selected. These
plans contain exclusions from and limitations of coverage.
Please see the product brochure for more complete
information, as well as information about terms of
renew ability, preexisting conditions, out-of-network
penalties, and notification requirements. Plans
are subject to health underwriting. To be considered
for reimbursement, expenses must qualify as covered
expenses. Expenses are also subject to reasonable
and customary limits, unless you use a network, and
all other policy provisions, including determinations
of medical necessity.

Coverage
Synopsis:
The
Regence HSA plans have 3 options for individuals
and families. The Regence HSA Plan is a tax-sheltered
savings account, which must be paired with a qualified
high deductible health plan (HDHP). Regence provides
your HDHP coverage. You can choose any financial
institution to have an HSA account. One of the benefits
of having an HSA plan is that it allows you to set
aside tax-deductible, interest-earning funds to
help cover your HDHP deductible, coinsurance, and
other qualified medical expenses. Any unused funds
roll over from year to year, and can later be used
for retirement.
The
Regence HSA Plan has individual deductible options
of $1,500, $2,500 and $5,000; and family deductible
options of $3,000, $5,000, and $7,000. The plan
offers 80% co-insurance after deductible, with a
maximum out-of-pocket (OOP) of $5,000 for individuals
and $10,000 for families. The OOP is only applied
to In-Network providers. The deductible is waived
for office or clinic visits for preventative care.
The plan will pay 80% of costs and you pay 20%.
Preventive
Care Services include Professional exams for adult
and children including routine diagnostic tests,
such as lab and x-rays. Also included are screening
procedures, such as Pap smears, prostate, mammography
screening, test of the stool for occult blood, bone
density scans, sigmoidoscopy and colonoscopy. Immunizations
are also included for childhood diseases and adult
immunizations for influenza, pneumovax, hepatitis
A and B, Rubella, diphtheria, tetanus, toxoid, varicella,
and measles and mumps.

Rates
are the same throughout Utah, and don't change by
zip code or effective date. Though rates may
change at any time, readjustments typically occur
on January 1st of each year.
The
premium can be paid monthly, quarterly, semi-annual,
or annual billing, or a monthly bank draft. The
bank draft will occur on either the first or the
sixteenth of each month. The initial premium
can be paid with a check or credit card.

Network:
The
Traditional (Regence BlueCross BlueShield of Utah)
and ValueCare Networks have lots of providers, so
yours is probably one of them.

Underwriting:
After
receiving a completed application, Regence HSA will
determine premium by the plan selected, health status,
gender and age of the adult insured, and the number
of children, if any, covered by the policy. If the
information received is accurate and complete, it
will normally take less than ten (10) working days
to review. Acceptance of your application is based
on your health and prior insurance coverage.
After
review of your application rates may change or remain
the same as quoted. If the rate does change it will
depend on certain health conditions that will be
accepted at a higher rate level, and you will be
given the option to accept the new rate or withdraw
your application. Some applicants may be denied
coverage for failure to meet Regence underwriting
requirements.

Effective
dates:
Your
coverage effective date will be on the first of
the month after your application was reviewed and
accepted. The effective date could be postponed
if there is a delay in accepting your application.
If this occurs, you will be notified by the underwriter.

HSA
Administrator
Regence
HSA can work with any federal institution HSA bank
account. For most of our clients we recommend that
you establish your HSA independently from your insurance

Regence
HSA has also teamed with Regence Financial Services,
powered by BlueBank, to set up HSAs for their policy
holders. This allows for convenience and one point
of contact. If you choose to combine a BlueBank
account with a Regence HSA plan you will receive
