Humana is
one of the most popular PPO carriers that we work with. The coverage
is strong, the rates are usually very good, and there is a very large PPO
network. For these reasons we also recommend Humana for our clients
who are seeking to add dental to their insurance coverage.

Dental
Plans at a Glance
Humana
offers three separate dental coverages to choose from: the Dental
Preventative Plus, the Dental C550, and the Dental
Discount. All three plans avoid the often painful underwriting
process, and as such, you need not worry about being declined due to preexisting
conditions, and you will know in advance exactly how much you will be paying.
The Plus plan is available at $16.99 per month, the C550
plan is $16.00 per month, and the Discount plan is $10.95
per month. A one time enrollment fee of $35 is required.
Dental
Preventative Plus - with the Preventative Plus plan, you are
not assigned a Primary Dentist, instead, you are free to choose from any of
the over 130,000 dentists in Humana's PPO network. There is no co payment
for office visits, and most preventative services are covered at 100% before
your annual deductible. After a $50 deductible ($150 for a family),
most basic services are covered at 60%*, and you receive an average discount
of 28% on major services. This plan has a $1000 maximum per person per
year. You should contact Humana for pre-authorization prior to any procedure over $300.

waiting period applies for basic services.
Dental C550
– the Humana Dental C550 plan focuses on maintaining oral health,
prevention, and cost containment. You may see your Primary Care Dentist
(chosen at time of application) as often as necessary. There are no
yearly maximums, no deductibles to meet, and no waiting period. While
most diagnostic and preventative services cost you nothing, charges for common
services performed at your Primary Care Dentist include: $0 examinations,
$0 x-rays, $10 office visits, $30 fillings (one surface), and $35 extractions.

Dental
Discount – this plan is for our clients who are looking for
a minimal amount of dental coverage. The Dental Discount plan allows
you to get between a 20 and 50% discount on most dental services at in network
providers. Average prices for common procedures include: $26 examinations,
$75 x-rays, $50 cleanings, and $67 dollar fillings. See brochure
