How to Apply

Please note that all applications will be submitted in the name of Wiley Long, the president of HSA for America.  If you have already been working directly with one of our advisors, then that person will be noted in our system as your personal contact.

Before you apply, be sure to view the Time brochure.  It contains important information regarding benefits, exclusions, limitations, renewability, and other terms of coverage.  

The Instant Quote page calculates Preferred Rates.  If you do not think you qualify for Preferred Rates, please contact us for an exact quote for your situation.

The fastest and easiest way to get your coverage in force is to Apply Online, using the link above.  Gather the following information before you start the application in order to speed the process:

1. Your doctors’ names, addresses, and phone numbers
2. Medications you currently take or recently took, including dosage and frequency
3. Dates of hospitalization
4. The name of the plan for which you are applying
5. Your requested effective date
6. Your credit card information to cover the initial premium

If you do not wish to Apply Online, there are two other ways to apply for any Time Insurance plan:

1. Visit the Time Assurant site to receive an instant quote and then proceed to Apply Online.
   
   
2. The Time Tele-App fax application is designed to be the fastest and easiest way to apply over the phone.
   
   
   • By using the Teleapp process you may apply with Time Insurance and receive an answer in as quickly as a week.  No payment is required with your application.  You will be billed once you have been approved.
     
     
   • To qualify for this accelerated application process, you must first be able to answer NO to a series of nine health related questions.  View the nine questions.
     
     
   • If you were able to answer NO to all nine questions, please view the Teleapp Application.  When the Adobe Acrobat reader opens with the application, use the menu bar at the top to save the application to a file or simply print it out right from the reader.  (You may also contact us and we will send you an application in the mail or by fax.)
     
     
   • Complete the application and fax it to us toll free at 1-866-284-0082.  You may also mail it us at our address below.  Once your application has been received, we will contact you with the number to call Time Insurance for a Personal Health Interview via telephone.  Depending on your medical history, this can take between 10-20 minutes to complete.
     
     
   • Once you have completed your interview process, the underwriter will instruct you as to what comes next.  If Time does not order medical records for your file, you will usually receive a decision on your application within 7 days.  If medical records are ordered, Time will usually issue a decision on your application within 7 days of receipt of your records from your doctor.
     
     
3. If you were unable to answer NO to any of the nine questions under the Teleapp process, you are still eligible to apply for any plan by filling out the Time application and sending it in.
   
   
   • Select the correct state application here.  When the Adobe Acrobat reader opens with the application, use the menu bar at the top to save the application to a file or simply print it out right from the reader.  You may also fill out our Application Request Form and we will send you an application right away.
     
     
   • Simply follow the instructions on the first page to complete the application.  Please note: make sure to clearly indicate on your cover letter which plan (One Deductible, Right Start or Right Start HSA) and which deductible you are applying for, as the Time application in most states does not give you any space to list this information.  
     
     
   • Send the completed application along with first payment to our address below, or fax it to us toll free at 1-866-284-0082.  While you may submit the first month's premium along with your application, you also may choose to apply on a C.O.D. basis and have Assurant bill you for your initial premium after you have been approved.  You may also choose to pay via monthly bank draft.
