To apply for
the Value Health Plan, fill out the Application, the Consumer Form, and the
Billing Form, and mail them to us along with a check for the first months
premium, payable to GEM Administrators.
Please
note that this plan will not cover anyone that has in the past 24 months had
chest pains, disease or disorder of the heart, liver, kidneys, or lungs, uncontrolled
high blood pressure, albumin or sugar in urine, uncontrolled diabetes, cancer,
tumors, or ulcers.
